Tarin Kowt (GEM) Ward at Repat Health Precinct

Geriatric Evaluation and Management (GEM) units provide comprehensive and responsive assessment, management, and care planning. We work in consultation with the older person, their family, carer, and current community. We use evidenced-based principles and interdisciplinary assessment to support restoration and rehabilitation, as well as management of medical conditions.

Services

We offer 7-day medical, nursing, and allied health support with the following focuses:

  • Cognitive status
  • Psycho-social assessment
  • Nutrition (malnutrition and micro-nutrient deficiencies, obesity)
  • Mobility, balance and exercise tolerance
  • Environment, daily activity patterns and function
  • Basic check for related issues such as swallowing, vision and dental
  • Medication interactions.

Referral information

Any inpatient service within SALHN  may submit referrals to us, or other networks may refer on behalf of a patient residing in the southern catchment area.  All referrals are triaged by GEM Triage Nurse Consultant.

Admission criteria for GEM patients:

  • Medically stable
  • Age >65 years, or >45 years for Aboriginal/Torres Strait Islanders or younger onset dementia patients.
  • Geriatric syndromes (falls, functional decline, frailty, malnutrition, reduced mobility) OR dementia, delirium or cognitive impairment
  • Requires assessment under a geriatrician and multidisciplinary team
  • Requires short-term restorative care
  • Demonstrates participation in therapy and has potential for functional gain to achieve optimal discharge destination
  • Palliative care needs specific to older people
  • Agreed goals of care
  • Completed 7-STEP Pathway
  • Patients on Level 1 or 2 ITO can be admitted

Exclusion criteria

The following factors will exclude a patient from being admitted to Tarin Kowt if they are:

  • Not medically stable
  • Patients who may require ICU or monitored bed care
  • Requiring complex and intensive clinical nursing requirements and/or multiple medical devices managed in an acute ward
  • Requiring an alternate rehabilitation, hospice, or acute psychogeriatric program
  • Requiring consultation from other specialist services not provided at Repat Health Precinct or Noarlunga Hospital.
