      • Specialised multi-disciplinary community care for those with complex mental health needs, with treatment focussed on psychosocial rehabilitation and recovery.
      • Collaboration with individuals open to the service, their carers and aged care service providers within the southern region.

    • Providing short term in reach to residential care facilities to improve quality of life and reduce avoidable hospital presentation.

Older Persons Mental Health can answer general service-related questions between 9.30am to 4.00pm, Monday to Friday (excluding public holidays).

Referral information and eligibility

  • Individuals 65+ years old (50+ years for indigenous people) experiencing an acute phase of their mental health conditions.

  • Individuals requiring an assertive multi-disciplinary treatment approach.

  • Individuals under the age of 65 years who develop early onset dementia with associated psychiatric symptoms who are not open to another mental health service.

  • General practitioners and preferred health care professionals can complete the relevant referral form below.
