New NALHN Outpatient Referral Hub Open!

There is now one fax number and e-mail address for all your referrals to our Specialist and Outpatient Services.  For further information please the Frequently Asked Questions (FAQs) and download our Outpatient Referral Hub poster to print and display at your clinic.

How to refer

All referrals to our Specialist and Outpatient Clinics can be made through the NALHN Outpatient Referral Hub.

We accept both fax and email referrals.

NALHN uses the SA Health Clinical Prioritisation Criteria (CPC), which has been developed to provide clinical information and guidance about essential referral criteria. We recommend reviewing the CPCs to ensure your referral contains the information required for assessment or review.

Once you have determined the service or clinic you wish to refer to, you can send to:

Fax: 1300 927 043
