How do I receive discharge letters for my patients?

Please ensure your contact details are correctly recorded in the Health Provider Registry as this database is used as a reference for GP contact details including current practice and preferred delivery method (eg fax, electronic delivery, etc). Incorrect information on the Health Provider Registry may lead to SALHN correspondence being misdirected and clinical handover being compromised.

Previous discharge summaries may have been uploaded to the patient’s MyHealthRecord. Discharge letters can also be requested directly from SALHN:

How do I obtain old records/results for my patients?

Previous discharge summaries, SA Pathology and SA Medical Imaging results may have been uploaded to the patient’s MyHealthRecord. Outpatient clinic letters are not routinely uploaded onto MyHealthRecord

Previous records and results can be requested from SALHN:

How do I refer patients to a specialist outpatient clinic?

You may also find additional support to help you manage your patient through other home-based or ambulatory support programs, specialist advice lines or Healthpathways SA, which may assist while the referral is being triaged and/or the patient is waiting to be seen.

How do I check my patient's outpatient appointment?

Please contact the relevant clinic or contact the Outpatient Support Service Liaison Nurse.
