EOI — Medication Management at Transitions of Care Study
The SALHN Department of Clinical Pharmacology are undertaking a research project exploring clinicians’ experiences of medication management at transitions of care.
- The study is looking for GPs involved in medication management at transitions of care following an acute hospital admission in SALHN.
- Participants will be invited to attend a 90-minute focus group or interview (via MS Teams) and two 90-minute co-design workshops over the next 12 months.
- Participants will be reimbursed for their time.

The webinar was supported by the RACGP and includes information about the clinical benefit of CPCs for GPs, referring clinicians, consumers and the broader healthcare system, and also gives real life examples of how to use the CPCs.
