We bring the care to you

My Home Hospital is a South Australian initiative that brings hospital care to people in the comfort of their own home, instead of having to go to hospital. This includes people who live in residential aged care facilities or supported accommodation settings.

The service will be available 24 hours a day, 7 days a week across metropolitan area and from July 2021 also the outer suburbs of Adelaide. Admission to My Home Hospital will be based on consumer choice and the My Home Hospital eligibility criteria.

What will happen if I am admitted to My Home Hospital?

Services in My Home Hospital reflect the care you would receive in a regular hospital. Care is provided by a team of medical, nursing and allied health staff.

The team will meet with you and work out a treatment plan to meet your needs. You will receive a visit at least once per day while you are admitted to My Home Hospital. If you need additional tests, these will be arranged for you and help will be available 24/7 if you or your loved ones are worried or need to ask a question.

What can be treated?

A range of clinical conditions can be safely and effectively managed in the home. Some of the conditions that can be treated in My Home Hospital include cellulitis and lung, kidney and urinary tract infections. You can always ask your nurse or doctor if My Home Hospital might be right for you.

Who are we?

My Home Hospital is delivered by Wellbeing SA in partnership with Calvary-Medibank.
Wellbeing SA is a new State Government agency with a long-term vision to create a balanced health and wellbeing system that supports improved physical, mental and social wellbeing for all South Australians. This includes supporting models for care in the community – such as My Home Hospital.

How was My Home Hospital created?

Wellbeing SA has been engaging with the community through a series of workshops and interviews to identify what is important for them in situations where health care is delivered in their home. This includes understanding the needs of older people, people living with disability, Aboriginal and Torres Strait Islander people, the LGBTI+ community and people from culturally and linguistically diverse backgrounds.

We have also worked with medical professionals and members of the community through a series of co-design workshops to ensure that the principles developed by the community are incorporated into the service.
