The Freedom of Information Act 1991 (SA) gives members of the public a legally enforceable right to access information held by the South Australian Government, subject to certain conditions.

Each of SA Health’s agencies operates separately for the purposes of Freedom of Information (FOI) legislation. For general information, including costs, where to apply, and how documents can be accessed, see the Freedom of Information page.

There is a $42.00 FOI application fee, some requests have an additional processing charge.

An FOI Officer will advise you of these charges once they receive your application and begin processing it.

If you are the holder of a current concession card, or if you can satisfy the agency that the payment of the fee or charge would cause financial hardship, the agency must waive or remit (reduce or refund) the application fee.

These proofs should be submitted along with your application.

How to apply

When making an FOI application, you will need to provide enough information to enable the correct documents to be identified. If you are uncertain, it is recommended that you contact the relevant agency’s FOI Officer for assistance.

Advice about how you would like to access the documents should be included in your FOI application.

If you are requesting access to another person’s documents, living or deceased, or are an agent requesting access to documents of a third party.

You will be required to provide the appropriate authority to do so by completing and submitting the EFNLHN Authority for Access to Documents of a third party (PDF 194KB).

A completed application and relevant attachments may be lodged by email to Health.EFNLHNFOI@sa.gov.au or directed to any Hospital or Health Service within the Eyre and Far North Local Health Network.

Fee payments may also be made by:

  • In person payment at any EFNLHN hospital or health service, or
  • By calling the EFNLHN FOI Unit on 0466 549 780 to arrange payment via Electronic Funds Transfer (EFT) or credit card payment over the phone.
