SALHN Acute Diabetes Education Service at Flinders Medical Centre and Marion GP Plus

The SALHN Acute Diabetes Service is available to people with diabetes and their families.

We provide inpatient and outpatient services across 2 sites: Flinders Medical Centre (FMC) and GP Plus Marion. Patients seen at FMC may be referred to other regional sites, if it is more convenient and clinically appropriate.

Services

Services are available for inpatients with newly diagnosed or pre-existing diabetes and outpatients requiring education and management (criteria based).

Our Diabetes Service is able to help you if you have:

Type 1 diabetes (child, adolescent, adult)
Complex Type 2 diabetes (child, adolescent, adult) requiring insulin
Gestational diabetes (diabetes in pregnancy)
Pre-existing diabetes in pregnancy

SALHN Diabetes care is:

Committed to patient education and self- management of diabetes
Provided by specialised medical and nursing staff supported by education, teaching and evidenced based research
Focused upon diagnosis and chronic disease management that optimises long term health outcomes

Educational programs

Educational programs are negotiated according to patient needs and may include:

Lifestyle advice/risk factor reduction eg healthy eating /activity
Psychosocial adjustment / problem solving skills
Blood glucose monitoring
Blood ketone monitoring
Continuous glucose monitoring
Medication management including injecting insulin
Insulin pump therapy
Insulin titration service (criteria based)
Gestational diabetes group education program
Diabetes in pregnancy
Complication screening /prevention of acute and chronic complications
Sick day management
Hospital avoidance intervention

Referrals

Referrals to the SALHN Acute Diabetes Education Service can be made by your local doctor. (GP) or Endocrinologist. You may also be referred to our service during your hospital stay.
