Residential Care Outreach Service at Flinders Medical Centre

The Residential Care Outreach Service provides care for residents of Residential Aged Care Facilities (RACFs) who have presented to hospital.

This team is able to provide medical advice, nursing care and advice regarding management of behaviours of concern, and support with decision making regarding care wishes and palliation where required.

The RCOT team is supported 7 days a week from the Emergency Department (ED) based Single Point of Contact (SPoC) Nurse Consultant provides a single point of contact for RACFs, SAAS, and GPs who require support with determining if transportation to hospital is required, and advice regarding alternative care options.  The SPOC nurse consultant can assist with advice and on referral to the right service for the situation that presents.

Where additional direct support is required, this will be provided by either:

  • The CARE service: 7425 0340
  • Hospital at Home (H@H) via inpatient team
  • My Home Hospital (MHH) via inpatient team, ED team, or CARE
  • Metropolitan Referral Unit (MRU) via inpatient team, ED team, or CARE

Eligibility criteria

Residential aged care facility residents over 65 years who have presented to or who are at risk of presenting to hospital.

Residential Aged Care Outreach Team (RCOT)

  • Geriatrician-led Medical Team with visiting RCOT Nurse Consultants.
  • Medical Team: Weekday medical cover - 8.00am to 5.00pm, available weekend mornings    Note: AMU Medical Team will provide in-hospital cover outside these hours
