High Risk Foot Service at SALHN

The Southern Adelaide Local Health Network (SALHN) High Risk Foot Service provides a multi-site high-risk foot service for individuals at risk of/with current foot ulceration or amputation.

The service implements evidence-based assessment and management of high-risk foot complications with the aim of reducing avoidable hospitalisation and amputation.

Referrals

You can complete the High Risk Foot Service Intake Referral Form (PDF 166KB) provided, use the templates for GP systems below or send your own version to Health.SALHNPodiatry@sa.gov.au or fax to (08) 8204 5020.

Referrals should include:

  • patient details - address, date of birth and contact phone number
  • reason for referral
  • relevant clinical history of the patient
  • list of current medications
  • any risks to staff
  • your details.

Referrals are triaged daily and allocated to services based on inclusion criteria.

Referrals are accepted from medical, nursing, and allied health.

Inclusion criteria

Urgent

  • Foot wound deep to tendon/bone
  • Foot wound in the absence of pedal pulses
  • Foot wound not healing after 4 weeks of appropriate treatment
  • Known or suspected acute Charcot neuroarthropathy
  • Local foot infection (eg cellulitis/osteomyelitis)

Please refer directly to the closest Emergency Department for:

  • Foot wound with systemic signs of illness (eg sepsis)
  • Critical Limb Ischaemia

Semi urgent

  • Foot wound with no signs of clinical infection
  • Foot wound with non-interventional management plan
  • Chronic Charcot neuroarthropathy management
  • Subacute inpatient care including gross neglect
  • Identify as Aboriginal or Torres Strait Islander
  • On Dialysis or with end stage renal failure

Individuals are transferred between sites based on care needs. This allows for both step down and escalation pathways.

We do not provide ongoing services, general foot care or footwear provision.
