Residential Care Outreach Service at Flinders Medical Centre

The Residential Care Outreach Service provides care for permanent residents of Residential Aged Care Facilities (RACFs) who have presented to  Flinders Medical Centre, Noarlunga Hospital, or the Urgent Care Hub — Repat. This team is able to provide medical advice, nursing care and advice regarding management of behaviours of concern, and support with decision making regarding goals of care.

The Residential Care Outreach Team (RCOT) is supported 7 days a week by the RCOT Nurse Consultant. The RCOT Nurse Consultant provides comprehensive geriatric nurse care to ensure continuity of care through the patient's hospital journey and return to the community. They are the 'inpatient outreach' arm of the service, conducting site visits to assist patients in their return to RACFs or home for palliation and more complex geriatric needs.
