How the are Voluntary Assisted Dying Clinical Portal is protecting the privacy of all individuals
Only authorised medical practitioners and SA Health employees participating in the voluntary assisted dying process can access the Clinical Portal.
Medical practitioners can only access the Voluntary Assisted Dying Clinical Portal if the Voluntary Assisted Dying Operations Team verifies they are eligible to act as a Coordinating or Consulting Medical Practitioner under the Voluntary Assisted Dying Act 2021 (the Act).

Patients cannot access or submit a request to access the Clinical Portal.
All forms required to access voluntary assisted dying must be submitted by the patient’s Coordinating or Consulting Medical Practitioner.
Patients may choose to complete and submit a Prepare for Assessment Form in the Clinical Portal to prepare for an assessment to access to voluntary assisted dying.

In exceptional circumstances, for example, in the case of a system outage, medical practitioners can contact the Voluntary Assisted Dying Operations Team for support to submit a form.

Yes, medical practitioners can select to “Save and return later” to save progress and resume filling out the form in future.
By completing and submitting a form in the Clinical Portal, medical practitioners fulfil their legal obligation to give a copy of that form to the SA Voluntary Assisted Dying Review Board under the Voluntary Assisted Dying Act 2021.
When you submit a form in the Clinical Portal, you will receive an email to your nominated email address confirming that the SA Voluntary Assisted Dying Review Board has received the form.
We recommend you keep a copy of the email for your records.
You can also view and download any forms you have previously submitted in the Clinical Portal.
