Your feedback helps us to make improvements to the quality of care you receive.

Ask staff for an interpreter if you need one.

Step 1 Talk to a staff member

If you want to provide feedback or raise a concern, please talk to a staff member.

Step 2 – Contact the Consumer Feedback Team

If they haven’t answered your concern, please contact the Consumer Feedback at the health service.

Step 3 – Seek support from an independent service

If you are not happy with the response to your feedback, you can contact:

Health and Community Services Complaints Commissioner (HCSCC)

If your feedback is about public hospitals or health services, private and non-government health and community services.

Aged Care Quality & Safety Commission

If your feedback is about the care of person in an aged care service.

National Disability Insurance Scheme (NDIS) Quality and Safeguards Commission

If your feedback is about the care of person a disability.
