Change services under an existing health facility licence
This application is used for updating an existing licence to add, remove, or modify prescribed health services.
Key timelines
- Applications are generally processed within 6 to 8 weeks from the date all required documentation is received
- Approval timeframes may vary depending on the complexity of the application and the outcomes of inspections and any associated remedial actions
New services must not be provided until formal approval is granted.
Application process
- Prepare application – Gather all required documents to support the proposed service changes
- Submit application – Complete and submit the online application form with supporting documentation
- Pay application fee – An invoice will be issued. Payment must be made before the application is reviewed
- Assessment and review – The application is checked for completeness. Additional documents may be requested if needed
- Site inspection (if required) – The facility may be inspected to assess its suitability for the proposed services
- Approval of service changes – Once the assessment is complete, the service changes are approved, and the licence is updated
- Licence issued – A new licence may be issued depending on the nature and scope of the changes

Private Hospital
- Application to change services provided - $2,837
- Additional inspection of premises - $2,837
Private Day Procedure Centre
- Application to change services provided - $1,702
- Additional inspection of premises - $1,702
Invoices are issued after application submission. Payment options will be detailed on the invoice.
* If an inspection is required, the inspection fee is included in the original application fee. Additional inspections due to incomplete work or remedial actions will incur extra charges.
