Transition Care Provider Panel
The Transition Care Provider (TCP) Panel is a panel of approved service providers engaged by SA Health to provide Transition Care Programme and Care Awaiting Placement programme services.
The Transition Care Programme provides short-term care that seeks to optimise the functioning and independence of older people after a hospital stay in assisting them to return to their home and avoid being prematurely admitted to residential aged care.
The Care Awaiting Placement programme seeks to enable eligible older people to safely transition from hospital to an interim placement in residential aged care where they receive support to finalise their arrangements for a long term placement..
Without these services, older people may experience inappropriate lengthy stays in a public hospital.
The TCP Panel is an essential component of the overall SA Health priority to improve the health and wellbeing of older people and alleviate demand on public hospitals.
How to join the TCP Panel
SA Health is now taking applications from providers who are interested in providing Transition Care services and Care Awaiting Placement services. This invitation to supply applies on an ongoing basis. . Upon submitting an application, a provider will be assessed against a set of evaluation criteria..
Under the Transition Care Programme, service volumes are not guaranteed with Service Agreements between the Local Health Networks (LHNs) and the providers once the LHNs at the point of LHNs inviting providers to supply services. To apply to join the TCP Panel please email Health.NGOPerformance@sa.gov.au who will then issue the application paperwork for completion.
