Aged and Extended Care Services (Geriatric Medicine) at TQEH
The Queen Elizabeth Hospital (TQEH) specialist geriatrics service provides for a full range of specialist geriatrics services in and out of hospital and is also home to the following:
- an academic centre of excellence, the National Health and Medical Research Centre of Excellence:
Trans-disciplinary Frailty Research to Achieve Healthy Ageing
The service has pioneered well known initiatives such as the Dignity in Care program and the Mindful of Dementia Day.
The aim of the clinical and academic service is through collaborative clinical practice, research and training to deliver positive outcomes for the health and wellbeing of older people.

- To provide for training, research and mentoring in geriatric medicine and gerontology

Services provided
Hospital — at TQEH, we provide:
- for inpatient care to consumers of Central Adelaide Local Health Network (CALHN) through our Geriatric Evaluation and Management (GEM) Unit.
- for a whole of hospital liaison and consultation service through our GEM Liaison and orthogeriatrics service.
- consultative services to the inpatient psychogeriatric medicine service at the TQEH.
For our community, we provide:

for outpatients clinics focused on geriatrics health issues including dementia diagnosis and management, falls assessment and prevention and movement disorders.
- for community geriatrician assessments upon requests by general practitioners across CALHN.
- for geriatrician consultation service to the CALHN Transition Care Program and the CALHN Aged Care Assessment Team.
- outreach to Port Lincoln and Whyalla and work with the local teams there to provide for geriatrician consultation services upon requests by general practitioners.

How to make a referral
Admissions, referrals and appointments
General practitioners and other medical practitioners can make a referral to our service:
- by faxing a referral to our department: (08) 8222 8593 marking it attention to the Geriatrician, Aged and Extended Care Services.
Urgent referrals to the inpatient service:
- by general practitioners or other medical practitioners are to be made via our GEM Liaison Service and a call can be made to the geriatrician, GEM Liaison Registrar or GEM Liaison Nurse to discuss via our hospital switch board.

- by general practitioners or other medical practitioners can be made by discussing the case with the Community Geriatrician or Registrar and they are contactable via our hospital switch board.

We encourage referrals to include pertinent information about patient demographics, medical, function and psychosocial information.
