The ‘My health information for when I go to hospital is a free resource we have made to help support people with intellectual disability when they need to go to hospital. This will tell the hospital important information about you and how they can support you better. Ask someone you trust and knows you really well to help you fill out the form. This could be a family member, carer or guardian. If you need to go to hospital, you should bring the completed form with you.

The National Disability Insurance Scheme is called the NDIS. The NDIS helps people with disability, their family, carers and the community. The NDIS helps people under the age of 65 with permanent and significant disability get care and supports.
The NDIS provides reasonable and necessary supports that a person needs to live and enjoy their life.
To get services and support, you need to apply to join the NDIS.
