Advance Care Directives for adults - Order free resources for your patients

Advance Care Directives (ACDs) are not just for the elderly or those in palliative care. SA Health recommends completion of an ACD by all adult South Australians. 

Patients may nominate family or friends as their Substitute Decision-Makers: people they trust to ‘walk in their shoes’ and comply with their wishes. 

This patient-centred holistic approach is an advantage over the previous Enduring Powers of Guardianship, Medical Powers of Attorney and Anticipatory Directions, as decisions made will be based on the person’s wishes and not what others ‘think’ is best.
