Consumers, family and carers are encouraged to provide us with feedback to improve our health services. Critiques, suggestions or ideas and compliments are all helpful ways to achieve this.

If you have feedback that you would like to share, please ask your nurse or health professional for:

  • A feedback form (PDF 444KB)
  • If you write directly on the form it can be submitted to the Team Leader of the ward or service, or placed in the ward/unit suggestion box where available.
