Transition to Home

Transition to Home is a service delivered by the Department of Human Services in partnership with SA Health. It provides transitional community housing and support for eligible NDIS participants who no longer need to be in hospital.

Bright Transitional Units

Transition to Home has five new transitional units for NDIS participants with extremely complex support needs that require a more specialised setting. The Bright Transitional Units support this participant group to discharge from hospital while they wait for longer term arrangements or to prevent unnecessary admissions to hospital.
