Find out how you can get hospital care provided by a team of medical, nursing and allied health staff in the comfort and privacy of your home.
My Home Hospital will deliver hospital care in your own home instead of having to go to hospital. This includes people living in residential aged care facilities and supported accommodation settings.
The service will be available 24 hours a day, 7 days a week across metropolitan and the outer suburbs (from 2021) of Adelaide.
Medical, nursing and allied health staff will visit you at home and may also use technology to monitor your care until you are well enough to be discharged.
Just like a physical hospital, you will need to be referred to My Home Hospital by a doctor or nurse practitioner.

Over time, additional conditions will be added to this list.
You need to be referred by a doctor or nurse practitioner to be admitted to My Home Hospital. This may be your regular GP, specialist doctor, an emergency department doctor or nurse practitioner.
Your referral will include contact information, important details about your condition and any medical history, information about any medications you are taking and anything else you want the My Home Hospital team to know about you.
My Home Hospital will be available to people living in metropolitan Adelaide, including people living in residential aged care facilities and supported accommodation settings.
My Home Hospital is planned to extend to include the Adelaide Hills, Southern Fleurieu, Gawler and surrounds from July 2021.
