The CO-ACT Program supports older adults in their transition from hospital care to community living.
Designed for those who have completed their hospital stay but are awaiting placement in Commonwealth Home or Residential Aged Care, CO-ACT offers a temporary, welcoming environment with professional, experienced staff to support day-to-day needs.
A home-like, supportive space
In CO-ACT, consumers enjoy a more homelike environment than a hospital, with options for shared or private rooms (based on clinical needs), beautiful gardens, shared dining areas, activity rooms, and 24/7 nursing care.
This supportive setting fosters dignity and engagement as we help you or your loved one transition to long-term Commonwealth aged care.
Specialized, safe design
Our site includes high-contrast signage, adjustable furniture, and handrails, ensuring accessibility and safety for older adults, including those living with dementia.
Comprehensive, consumer-centred care
Our team of nurses, allied health staff, doctors and other support staff prioritise the needs of each individual, their family, and carers. This approach offers a broad range of services focused on respect, connection, and personalised care as we assist consumers in finding a suitable aged care placement in the community.
