Leaving hospital

We understand that being in hospital can be a worrying time.

From the start of your hospital stay, we begin planning for you to be discharged (to leave hospital) as soon as you are well enough.

When your hospital team decides you no longer need hospital care, you will need to leave hospital for ongoing support or to continue your recovery. Your hospital team will talk to you and your family, or your carer or support team, about the next place for you to go.

This next place might be:

  • Back home (with support if you need it).
  • Somewhere short-term while waiting for support services before going home, or, a long-term care option.
  • An aged care home.
  • Disability accommodation.
  • A respite facility for a short break.
  • Somewhere for rehabilitation.
  • Another hospital outside of Adelaide or in a regional area closer to home.
  • A private hospital.

You cannot stay in the hospital once your hospital team say you are ready for discharge.
