To contribute to improved health and wellbeing outcomes among Barossa Council residents by utilising social determinants of health based online social prescribing tool to link local people to local community based services and support.

Barossa Council community region.
External partner and their role:
Flinders University: to provide support on the project implementation, validation of the Steps to Better Health questionnaire, coordination of survey analysis and report, and support development of project evaluation pre- and post.
Project overview:
Establish social determinants of health (SDH) based online social prescribing tool for primary prevention purposes, by linking local people to local community based services and support via the Barossa Cares website. Use of the tool will be promoted to both key stakeholders and the community generally. The project will conduct pre and post surveys to assess change in social determinants of health, change in health and wellbeing status and change in service usage since being involved in the project. It will facilitate focus group discussions to evaluate the experiences and acceptability of the tool and undertake service mapping to identify areas of need and service gaps. The project will also aim to establish a Social Prescribing Community of Practice to support and facilitate social prescribing in the Barossa region.
