Root Cause Analysis training

Root Cause Analysis (RCA) is a method of investigating incidents. The purpose of a Root Cause Analysis investigation is to identify system issues that contributed to or resulted in the incident occurring and to provide recommendations for measures to prevent a reoccurrence of a similar incident.

Training components

The RCA training consists of:

  • completion of five Institute for Healthcare Improvement (IHI) Patient Safety and Quality Improvement Open School Modules prior to attending the training session
  • background reading and the completion of RCA Pre training practical exercises
  • attendance at a half-day training session, conducted on-site at the Local Health Networks in small groups (six to 20)
  • completion of the post training workbook, which enable’s participants to demonstrate their understanding of the concepts presented during training.
