The open disclosure of incidents is an important part of the incident management process and is essential to a patient centred model of care
Root Cause Analysis (RCA) is a method of investigating incidents. The purpose of a Root Cause Analysis investigation is to identify system issues that contributed to or resulted in the incident occurring and to provide recommendations for measures to prevent a reoccurrence of a similar incident.
RCA training is available for clinical and administrative staff in SA Health. Training is jointly organised by the Department for Health and Ageing, Safety and Quality Unit and the Local Health Networks (LHNs).
