The Model of care for community mental health services (16-65 years) describes our vision, values and guiding principles. It provides a description of our current services across CALHN mental health and the critical role that the tertiary Community Mental Health services provide to our consumers and carers.
Consumers are at the centre of our care delivery and have participated in the development of this model of care.
Our staff are the foundation of how we provide safe, high quality consumer centred care. Staff are recognised for their skills and abilities and will be supported to continually build their capabilities.
The model of care is the first of a suite of strategic documents that will provide a framework for the guiding principles, core business, priority areas, service provision, governance, operational and strategic planning priorities and directions.
The aim of the model of care is to build capacity and to improve the continuity of care and effectiveness of community services.
The Model of Care V2.0 (PDF 1MB) has been endorsed by the governing bodies and is now available.
