Metropolitan Referral Unit Information for GPs fact sheet

RDNS, in partnership with SA Health, has introduced the Integrum service for patients in metropolitan Adelaide (south of the Torrens), with complex chronic conditions in order to provide the extra supports that are often needed for this patient group to remain living safely at home.

The multidisciplinary team of doctors (including a geriatrician), nurses, case managers, non-dispensing DATIS pharmacists, exercise physiologist and social worker will support patients to work through the Flinders Chronic Condition Management Program, and to develop an advanced care plan if not already in place. The program promotes independence and social connections and provides patients with fit for purpose technology to support self-management and is committed to working with the patient’s usual GP, and respects the long standing existing relationship that GPs have with their patients.
