In just three months, the Rehabilitation Acute Flow Team (RAFT) has already supported 100 patients on their recovery journey — and they’re just getting started.

RAFT provides early, high intensity rehabilitation to help patients get moving while they’re still in hospital. By starting support sooner, the team helps people maintain strength, build confidence and plan a safe return home.

“The RAFT team works alongside patients to improve their physical function and independence, identify suitable equipment for home and support shared decision making around the most appropriate discharge pathway,” says Ben, A/Senior Manager, Rehabilitation Acute Flow Team.

RAFT works closely with acute care teams to support discharge planning and reduce delays in rehabilitation care. This collaborative approach helps improve patient flow and ensures people receive the right support at the right time.
