How to Conduct a Clinical Incident Review Guideline

Clinical incident review is a process of staff reflection about a patient incident to improve patient safety and build trust in the health service. The How to Conduct a Clinical Incident Review Guideline (PDF 666KB) provides information and tools to support the process of clinical incident review and aligns with the requirements of the Clinical Incident Management Policy (PDF 521KB).
Applicability
This guideline applies to all employees and contractors of SA Health; that is all employees and contractors of the Department for Health and Wellbeing (DHW), Local Health Networks (LHNs) including state-wide services aligned with those Networks and SA Ambulance Service (SAAS).
