Aboriginal Maternal and Child Health- Improving Continuity of Care

In 2023-24, SA Health, Aboriginal Health aims to implement a Program that will improve continuity of care for Aboriginal children 0 to 4 and mothers of Aboriginal babies across the SA Health system. This Program is the Continuity of Care Protocol Program (CCP Program), which is comprised of Continuity of Care Protocols (CCP) and an Enabling Framework.

Objective

The objective of CCP Program is to improve the quality of life of Aboriginal children 0 to 4 and mothers of Aboriginal babies by improving the performance of the health care system, with a focus on continuity of care.

The CCP integrate cultural components with opportunities to improve relational, informational, and managerial continuity across all sectors of the health systems and between government agencies.

Consultation

Stakeholder consultation and workshops were conducted during April – July 2022. These workshops included participants both Department for Health and Wellbeing and Local Health Networks, from the Aboriginal Community Controlled Health Organisation (ACCHO) sector, professional organisations and researchers. The workshops provided invaluable feedback on the options proposed to take forward and explore further as part of the Testing Phase of the CCP Program, and crucial insights on how we can work together to make sure this next testing phase sets the program up for success.

Testing

Aboriginal Health will test the components of the 2022-23 to ensure that the CCP program implemented in the following year is founded on rigorous, fit-for-purpose evidence. The plan for the Testing Phase (2022-23) is being developed during August and September 2022.
