Outpatient clinic letters are not routinely uploaded onto My Health Record.

How do I receive discharge letters for my patients?

Please ensure your contact details are correctly recorded in the Health Provider Registry as this database is used as a reference for GP contact details including current practice and preferred delivery method (e.g. Secure message delivery, fax etc). Incorrect information on the Health Provider Registry may lead to SALHN correspondence being misdirected and clinical handover being compromised.

Previous discharge summaries may have been uploaded to the patient’s My Health Record. Discharge letters can also be requested directly from SALHN:

How do I obtain old records/results for my patients?

Previous discharge summaries, SA Pathology and SA Medical Imaging results may have been uploaded to the patient’s My Health Record. Outpatient clinic letters are not routinely uploaded onto My Health Record.

How do I refer patients to a specialist outpatient clinic?

How do I check my patient's outpatient appointment?
