Rehabilitation in the Home (RITHOM) - 4th Generation Clinics

Rehabilitation in the Home (RITHOM) is an intensive home based rehabilitation program which offers early discharge from hospital.

Rehabilitation can be provided for up to four weeks depending on need. The service may offer the following services:

  • occupational therapy
  • physiotherapy
  • speech pathology
  • social work
  • nursing care, including managing wounds and medication
  • exercise physiology
  • dietetics
  • rehabilitation medical specialist review
  • close GP liaison
  • tele-rehabilitation using an iPad - equipment and internet connection provided
  • home environment assessment with recommendations for home modifications to promote safety and independence
  • personal care assistance so that people can regain their independence with showering and dressing
  • equipment - temporary free loan
  • access to neuropsychology, psychology, orthotics/prosthetics

People who would like to be assessed for Rehabilitation in the Home will:

  • currently be in hospital and have rehabilitation goals
  • have sufficient memory and reasoning
  • be motivated to participate
  • be medically stable
  • have a home environment that is suitable for therapy


Contact our rehabilitation coordinators prior to discharge as people are assessed for Rehabilitation in the Home while they are still in hospital.
