The Community Geriatric Service (CGS) provides a clinic and in-home assessment to support older people with complex care needs. The services includes:

  • geriatrician consultation
  • short term case management and support

Driving Fitness Assessment Clinic - 4th Generation Rehabilitation

Doctor and student assessing patient&aposs forearm and elbow in Driving Fitness ClinicThe Driving Fitness Clinic offers Medicare bulk billed services for people who need to be assessed for physical, visual, cognitive and psychological fitness so that they can continue or return to driving. Assessments are based on Ausroad licencing guidelines and a Certificate of Medical Fitness to Drive can be issued if required. The clinic can also refer to practical on-road assessment if appropriate.

Falls Clinic - 4th Generation Rehabilitation

The Falls Clinic assesses people above 65 years and people who identify as Aboriginal or Torres Strait Islanders who are above 45 years of age that are at risk of falls or falls related injury. Younger people are assessed if they are admitted to hospital as a result of a fall, are unsteady or have a recent history of falls (more than 2 in the last 6 months).

Fragility Fracture Service - 4th Generation Rehabilitation

The Fragility Fracture Service offers a Medicare bulked billed service for people age 50 and above who have sustained a fragility fracture (or minimal trauma fracture).

A rehabilitation physician, specialist nurse (fragility fracture liaison), dietician and physiotherapist will assess the patient.The focus of the service is identifying the fragility fracture, investigating the fragility fracture, and initiating treatment (pharmacological and non-pharmacological). 

The service provided may include:

  • Fracture risk assessment
  • Falls risk assessment
  • Dietetic focused assessment
  • Medication review and initiation of osteoporosis treatments
  • Investigations (pathology, bone density testing)
  • Physiotherapy (strength and balance)
  • Self-management education

Patients attend the fragility fracture service for two appointments, though may be referred on for further physiotherapy and dietetics, or other specialty rehabilitation services (ie syncope clinic, memory clinic, aged services, musculoskeletal) requiring further attendance at 4th Generation Rehabilitation clinics.

General Rehabilitation/post ICU/Disability - 4th Generation Clinics

General Rehabilitation/Post ICU/Disability Clinic provides public and private (Medicare bulk billed) assessment for:

  • older people who want advice on getting back to full function (eg from a broken hip or arm) after being in hospital and they haven’t fully recovered
  • adults who have been in intensive care or have had cancer treatment and lost walking ability and independence
  • adults who have a disability and want to know whether robotic treatment will help
  • adults who may benefit from access to robotics eg the Lokomat
  • adults who may benefit from a multidisciplinary program (3-6 weeks) Day Rehabilitation.

This clinic is not suitable for people who require physiotherapy only.

Memory Clinic - 4th Generation Rehabilitation

The Memory Clinic provides Medicare bulk billed services for people with memory difficulties, and manages conditions relating to dementia. 

The first clinic visit will take between 1-2 hours and includes memory testing with a nurse followed by a medical review. Memory assessments are also available at the Aged Care Clinic.

The Neuro-rehabilitation Clinic offers Medicare bulk billed services for people with chronic neurological conditions that have left them with a disability (eg difficulty walking). Conditions we assess include:

  • brain injury

  • cerebral palsy in adults
  • complex neurological conditions where advise is needed to maintain independence
