Freedom of Information (FOI) requests at Modbury Hospital

The Freedom of Information Act 1991 gives members of the public a legally enforceable right to access information held by the South Australian Government, subject to certain conditions.

How to make an FOI application

The Northern Adelaide Local Health Network (NALHN) has an Accredited FOI Officer based at Modbury Hospital. FOI Officers assess each application to determine which documents should be disclosed and which may be exempt under the FOI Act. It is highly recommended that you discuss any application with the hospital FOI officer before lodging.

When lodging an application you must:

  • apply in writing
  • pay the application fee of $34.25 or provide evidence of a concession card or other evidence as proof of financial hardship
  • provide proof of identity.

Application forms can be downloaded (below) or can be obtained from the main entrance of the hospital or the Freedom of Information Office.

Note: A request for access will be dealt with as soon as practicable, or within 30 (calendar) days of it being received.
