Respiratory Care Pathway for Southern Adelaide LHN
The Respiratory Care Pathway is a multi-disciplinary, integrated care service established to prevent potentially preventable admissions, re-admissions and offer supported discharge services for patients presenting to the Flinders Medical Centre.
The Respiratory Care Pathway works in partnership with the FMC inpatient teams and the respiratory integrated care service (RICS) as well as offering support to the GP and community primary care providers.
Interventions are provided in the patient’s home or in a designated outreach clinic at a GP Plus Centre. Services can also be provided to RACF’s as required.
- Airway Clearance program
Provides specialist respiratory physiotherapy for patients with COPD and bronchiectasis who require intensive airway clearance support to prevent clinical deterioration and potential complications.
- Home Enhanced Respiratory Rehabilitation Outreach (HERRO)
A home based Respiratory Rehabilitation program targeting patients transitioning from hospital to home following an acute inpatient respiratory admission. A case co-ordinated package of care is offered in the patient’s home which is physiotherapist led with support from respiratory nursing. Other allied health professionals are involved as required.
- Pulmonary Rehabilitation (Community based)
An 8 week course which provides an evidence based respiratory rehabilitation program for patients with chronic lung diseases. This team works closely with the Pulmonary Rehab Program based at FMC.
