Liver Service Pathway for Southern Adelaide LHN
The Liver Pathway is part of Intermediate Care Services (ICS). It consists of support relating to Chronic Liver Disease and Viral Hepatitis.
Chronic Liver Disease
The Chronic Liver Disease Service provides a case management model with specialist nursing support to assist patients with liver cirrhosis to manage their health in the community. All patients admitted to FMC with a liver related problem are screened and those with cirrhosis are assessed for admission to the Chronic Liver Disease Service.
This is a complex group of patients usually with more than one chronic disease. The Chronic Liver Disease Service offers telephone advice, organisation of urgent GP or specialist appointments, outpatient or home visits as a rapid response to deterioration and attendance at appointments for support and coordination of care.
As case managers, the program works alongside community and other key health providers to maximise patient health and wellbeing outcomes, providing early intervention that prevents hospital attendances.
Viral Hepatitis Liaison Service
The Viral Hepatitis Liaison Service facilitates the coordination of patient care between primary care physicians and hospital-based specialists with the aim being patient centred, holistic, safe, cost effective and culturally and geographically accessible services being provided to all people affected by chronic Hepatitis C within SALHN and expansion of targeted prevention programs.
Clinical practice consultants in Hepatitis C are responsible for facilitating the coordination of patient care between primary care physicians and hospital-based specialists within a shared care model of best practice. The key priority areas are expansion of access to effective treatments aiming at the main populations affected by Hepatitis C within SALHN and expansion of targeted prevention programs.
