Diabetes Services for Southern Adelaide LHN

Diabetes Care Pathway is a multi-disciplinary, integrated service with Flinders Medical Centre (FMC), Repatriation General Hospital (RGH), Noarlunga Health Service, endocrinologists, and ICS diabetes nurse educators, dietitians, psychologists, social Workers and podiatrists.

Services provided

  • Rapid access for management of acute changes in glycaemic control
  • Education and clinical management of newly diagnosed Type 1 diabetes in adults and adolescents 15 years and older, not requiring hospitalisation.
  • Education and clinical management support for pre-existing Type 1 diabetes in children in collaboration with FMC Paediatric Unit
  • Education and clinical management support for newly diagnosed adolescents and adults, as well as in pre-existing Type 2 diabetes in with HbA1c >8% (IFCC64mmol/mol)
  • Education for people commencing injectable diabetes medication
  • Commencement and management of insulin pump therapy
  • Education and training of personal Continuous Glucose Monitoring Systems (CGMS) and set up for short-term clinical monitoring to enhance treatment for patients
  • In-reach support and training for patients and staff at Noarlunga Hospital

Services not provided

  • Management of people without a diagnosis of diabetes (eg pre-diabetes / uncomplicated obesity)
