Community Complex Care Team (CCCT) at Southern Adelaide LHN

The Community Complex Care Team (CCCT) is part of the Intermediate Care Services Hospital Avoidance and Supported Discharge Care Pathway. CCCT is a Southern Adelaide Local Health Network initiative that supports people in the community who are at risk of avoidable admissions to hospital as a result of their chronic condition(s) or combined health and psycho social needs.

The small team of social workers and nurses take a case intensive management approach to better support care in the community and prevent unnecessary hospital presentations and admissions.

The team actively focuses their support on individuals who have had three or more avoidable ED visits or unplanned hospital admissions within the last 12 months, have been diagnosed with chronic health conditions and have complex health and/or related psychosocial needs.
