Hospital avoidance and discharge support services

Hospital avoidance services provide short term support for people who are at immediate risk of a hospital presentation. 

They can also provide a safe and supported environment for patients to leave hospital earlier than otherwise possible.

Services can be delivered in a person's home or in residential care.

What services are available?

The Metropolitan Referral Unit (MRU) provides a single point of contact for referral to a range of hospital avoidance and discharge support services.

This service can be used for clients living in the Adelaide metropolitan area.

Available support services include:

  • nursing/midwifery care
  • allied health services
  • palliative care support

Who can refer to the Metropolitan Referral Unit?

  • General practitioners 
  • Residential care facilities
  • SA Ambulance officers
  • Public metropolitan hospital staff
  • Community mental health teams

Service opening times

The MRU is open between 8:00am and 8:00pm, seven days a week.

Please note that the service is not an emergency response and usual processes for emergency care should be followed.
