Notification of adverse events

The reporting of Safety Assessment Code (SAC) 1 incidents, which includes sentinel events, is mandatory. They must be reported to the Department of Health within 24-hours of knowledge of the event by completing an adverse incident - report in brief (PDF 45KB) form.

SAC 1 incidents should also be reported to the SA Incident Management System (IMS) as soon as practicable by phoning 1800 668 439 (1800 NOTIFY).
