A Safety Assessment Code (SAC) 1 adverse event that involves a registered mental health service consumer, requires a Root Cause Analysis (RCA) to be undertaken. This is to be initiated by the health unit’s Mental Health Director.

Where an incident is unrelated to treatment or to the mental health service from which the person was receiving care (for example, death due to natural causes), a case note audit should be undertaken to determine whether a RCA would be beneficial.
