Designated authority

The designated authority is the general manager or chief executive officer of a health service entity, however described; or a person who is appointed by a health service entity to exercise the powers of a designated authority under this Part; or an authorised quality improvement body authorised committee under Part 7 of the SA Health Care Act 2008 (PDF 45KB).

Root Cause Analysis team

A Root Cause Analysis (RCA) team is a team appointed under section 69 SA Health Care Act 2008 to investigate a Safety Assessment Code (SAC) 1 incident or sentinel event. The designated authority appoints the team via Letter to Appoint a Root Cause Analysis Team (DOC 120KB) and Letter to Appoint a Root Cause Analysis Leader (DOC 120KB). The team is to consist of no less than three members.

Team leader

The leader of the RCA team must have completed a formal training course in Root Cause Analysis - Health Care Regulations (SA) 12(1)(b). The team leader reads and signs the Information for Root Cause Analysis (RCA) team leader to read prior to signing RCA agreement (PDF 133KB) and signs the Root Cause Analysis Agreement (team leader) (DOC 134KB).

Team member

At least one member of the RCA team must have a formal tertiary qualification in a health-related field relevant to the investigation - Health Care Regulations (SA) 12(1)(c). Each member of a RCA team must have knowledge and understanding of his or her obligations under the SA Health Care Act 2008 parts 7 and 8 - Health Care Regulations 12(1)(d). Each team member is provided with Information for RCA team member to be read prior to signing RCA agreement (PDF 133KB) and acknowledge their understanding by signing the RCA Agreement (team member) (DOC 133KB).
