Incident Management System

SA Health has implemented a computer software program for the purpose of capturing patient related incidents. The system allows for:

• incidents to be reported electronically and data to be available centrally for analysis
• voluntary reporting of patient related incidents and near misses to be notified by all health employees.
• incident investigation and continuous quality improvement activities to be carried out.

Incidents are notified through a central call centre 1800 668439 (1800 NOTIFY).

Incidents are notified by a phone call to the contact centre during the hours of 6.30 am to 10.30 pm, seven days a week.

Administrator

The local site administrator will assist in for providing access to Data Manager and Analyser to provide analytical reports.
