Conducting an investigation

As a matter of policy, notification of a sentinel event or Safety Assessment Code (SAC) 1 incident is required to be reported to the Department of Health within 24-hours of the health service entity being alerted to the incident.

Conducting a root cause analysis (RCA) should be viewed in tandem with SA Health Care Act 2008 parts 7 and 8 (PDF 45KB). It includes the legal requirements under Part 8 with which compliance is required to ensure the maintenance of the prohibition of disclosure of the information discovered during a RCA.
