The Freedom of Information Act 1991 (SA) makes provision for public access to official documents and records held by government agencies. The objects of the Act are, as far as possible, to allow members of the public access to information held by the government, and to ensure that records held by the government concerning personal affairs are accurate

Application to access documents

Below is the freedom of information application form that you can print out and complete:

If you prefer to write a letter requesting access to documents, your letter must:

  • specify that you are making an application under the Freedom of information Act 1991
  • include your name and your postal address in Australia
  • describe the documents you would like to access
  • specify whether the documents contain information about your personal affairs
  • include the application fee unless you are lodging an application to amend your personal records or you hold a concession card (proof will be required)
  • send directly to email or postal address below

Application to amend personal information

Under freedom of information you may make an application to have documents concerning your personal affairs amended if they are incomplete, incorrect, misleading or out of date.

Application fees and charges

Applications under the Freedom of Information Act 1991 (SA) for access to documents that are in the possession of the Southern Adelaide Local Health Network must be in writing and should be accompanied by a $34.25 application fee (GST exempt), as specified in the current application form. Fee exemptions may be provided for eligible veterans and pensioners with photocopied proof of eligibility.

Additional costs may be incurred in accordance with the Freedom of Information (Fees and Charges) Regulations 2003 (eg photocopying costs, reproduction of medical images, and time required to process the application).

Application to review a decision under FOI

Under s29 and s38 of the Freedom of Information Act 1991, if you are dissatisfied or “aggrieved” with certain decisions or “determinations” of the Southern Adelaide Local Health Network, you can apply for an internal review of the determination.
