If you wish to access your personal medical records it is most likely that this information is held at a public hospital or health service in one of the Local Health Networks above.

The Department for Health and Ageing (the department) is required to comply with the Freedom of Information Act 1991 (FOI Act) and its related regulations.To do this effectively, formalised systems have been put in place and are observed by staff across the agency.

Freedom of Information (FOI) applications lodged with the department are coordinated through the FOI team within the Office of the Chief Executive.
