Intermediate Care Services for Southern Adelaide LHN

Intermediate Care Services is a multidisciplinary community based service that aims to support hospital avoidance and earlier supported discharge for patients with chronic conditions and/or complex care needs within the SALHN region.

The services also support:

  • integration between hospital and community providers
  • a single point of entry for access to the full suite of Intermediate Care services as required
  • Episodic, criteria led interventions, prioritised for those at high risk of admission/presentation.
  • comprehensive screening and assessment of health and psychosocial needs/risk factors
  • person centred care provided on site or in the home setting and supporting client education and self-management
