Open disclosure

SA Health is committed to the process of open disclosure. This involves an open and honest discussion with a consumer and their support person following an incident that resulted in unintended harm to them.

SA Health has developed an Open Disclosure Policy (PDF 162KB) which is consistent with the National Open Disclosure Standard.

The SA Health Incident Management Policy (PDF 260KB) and Guideline (PDF 216KB) has been updated to incorporate Open Disclosure.

Workshops and master classes focusing on clinical communication skills for open and honest communication with consumers following an incident are being conducted to enhance the effectiveness of the open disclosure policy. A number of tools and resources have also been developed to support a consistent approach.
